For events shorter than 400 meters, the Olympics uses a heats >> semifinal >> final format. Heats are swum by everyone entered in the event in the morning. The fastest 16 swimmers from the morning heats advance to the evening semifinals. The fastest 8 swimmers from semifinals advance to the finals, held the following evening.
The 400 freestyle and 400 individual medley use a heats >> finals format. All swimmers race in the morning heats, and the fastest 8 swim in the finals that evening.
The men's 1500 and women's 800 use a heats >> finals format. The heats are held on one day in the morning, and finals are held at the next day's evening session. All swimmers race in the morning heats, and the fastest 8 swim in the finals.
Relays use a use a heats >> finals format. The fastest 8 relays qualify for the evening finals. These final relays do not have to enter the same swimmers on them, relay teams are often comprised of different swimmers in the morning heats and in the evening finals (all swimmers that were on a medal winning relay get a medal, whether they swam in the relay in the heats or in the finals).
Finals are what counts for the gold, silver, and bronze. The top three swimmers in finals win the Olympic medals.
